Starter Documents

New Starters

Before starting your training with HEE, you must take the following preparatory steps:

  1. Complete the digital Form R Part A & Form R Part B. Please note that all documentation must be opened and completed using Adobe Reader. If the document is opened in an Internet browser or Mac PDF Preview, security protocols within the form will become invalidated and not allow you to complete your form. As stated in the Gold Guide, “All trainees must register with the appropriate Postgraduate Dean by obtaining and returning the Registration Form R” (6.21).
  2. Read & sign the Postgraduate Training Agreement.
  3. Attach all of the above paperwork alongside your latest ARCP outcome form to an email and send to: heee.edit@nhs.net. Your paperwork must be sent digitally. HEE will not accept any forms that have been printed and scanned or sent via post.
  4. Your paperwork will be checked by HEE. If it is incomplete or incorrect it will be rejected and you will be emailed with the specific reason. You must amend your form with the necessary corrections before you start your training post. If your paperwork has not been accepted by this time, you are not formally registered with HEE or your College and risk jeopardising your training.

Please note: All forms should be opened and completed using Adobe Reader. Please ensure you do not attempt to complete the forms in any PDF previewers, such as inside your internet browser or if on a Mac, Mac PDF preview. In order to download and open your forms in Adobe Reader on a PC, either right click the link and select "Save link as..." or click the link as normal and once opened in your internet browser select "Download". If on a Mac, please see our Mac specific guidance by clicking here.

If you are having difficulty completing any of the above steps, either view the comprehensive Form R guidance by clicking here or contact heee.edit@nhs.net and a member of the team will be happy to assist.

Wishing you every success in your future career!
