ELIGIBILITY:

Please Note:

Category 1 applicants have priority and HEE will support all category 1 applicants on the grounds of their health, disability or caring responsibilities, subject to available funds and training capacity.

Category 2 applicants are dependent on individual circumstances and the availability of suitable training placements.

Category 3 applicants can apply for LTFT as a personal choice and are not required to meet either category 1 or category 2 criteria.

This is fixed at 80% FTE. The standard period is 4 months, however, provided there is agreement between the trainee, employer and HEE Local Office the short term LTFT offer can be extended, and this may be appropriate to align with rotation dates. Although, trusts may permit 6 months.

It was only available to trainees in Emergency Medicine, Obstetrics & Gynaecology and Paediatrics. From August 2021 it will be available to all trainees after full implementation as detailed in table 1 below.

Applications:

Please find the link to the electronic LTFT form here.

All LTFT application forms must be electronically completed in full by the applicant and emailed to the LTFT mailbox at ltft.eoe@hee.nhs.uk. All submitted forms must be agreed by HEE in order for you to commence your requested reduced working arrangement.

Please note that LTFT posts cannot always be accommodated straight away.

Trainees will be informed of the LTFT Associate Dean decision by email, usually within two weeks of submitting their application form, providing no information or required evidence is missing.

Please ensure the form is submitted electronically to the LTFT mailbox  with all mandatory fields marked with a red asterisk (*) completed. Hand written, scanned or posted application forms will not be accepted.

Change in plan (Located on the same form)

If you wish to change the percentage of LTFT you are working then you must submit a Change in plan form which is located on the Form 1 document (you will need to use a blank copy).

There is no requirement to renew if you have put your LTFT end date as the same as your CCT date.

Please ensure you save the form locally and open as a PDF with Adobe Acrobat or Adobe Reader before completing. Do not begin completing the form if it has opened as a webpage or in PDF Previewer, as this will disable the electronic signature fields and deem the form invalid. Please see a link to download Adobe Reeder Instructions for setting up a digital ID and additional assistance for Mac users are also available at the bottom of this page. As an organisation we are moving towards going paperless and appreciate your assistance with this. Please ensure you carefully read the instructions on page 1 of the LTFT application form prior to completion.

Important Note:

Where an application is refused by HEE the applicant has a right of appeal. The training capacity of a training programme and service commitment will have to be taken into consideration. If a doctor in training has had their application to train LTFT declined, they may appeal in writing to the relevant Postgraduate Dean. The Postgraduate Dean’s decision will be final. It is recommended that you first attempt to resolve any issues informally via discussion with your TPD and the AD for LTFT (rowan.burnstein@addenbrookes.nhs.uk ).
