LTFT Process Flow Chart:

If you are on a Tier 2 Visa please make sure you read the guidance carefully before applying. A full list of FAQs can be found here.

 

Important note:

  • Please be reminded you are not permitted to commence training less than full time until you are in receipt of your fully approved Form 1 LTFT application.
  • Trainees will be informed of the LTFT Associate Dean decision by email, usually within two weeks of submitting their application form, providing no information or required evidence is missing.
  • Where an application is refused by HEE the applicant has a right of appeal. The training capacity of a training programme and service commitment will have to be taken into consideration.

*12 weeks’ notice must be provided (that means the application has to be approved 12 weeks before the start date). Please note it can take up to 28 working days to process your application, so please do not delay submitting your LTFT application form

Step by step process:

Step 1 (18 - 16 weeks in advance): Discuss with Training Programme Director (TPD) and Educational Supervisor (ES) detailing circumstances including dates/timetables.

Step 2 (18 - 16 weeks in advance): Trainee may complete LTFT application form and digitally sign. All fields are mandatory.

Step 3 (18 - 16 weeks in advance): Send to TPD to fully complete section 5 and digitally sign and also to ES to digitally sign the form .

If the trainee is in a GP Practice placement, a signature from the Practice Manager is required and also trainee may provide a brief proposed weekly timetable of their clinical and educational hours.  

Step 4 (16 - 14 weeks in advance): Once all the signatures have been added then the form can be sent to england.ltft.eoe@nhs.net

Step 5 (16 14 weeks in advance): A confirmation email will be sent from HEE once the form is approved by the Associate Dean.

All LTFT application forms must be electronically completed in full by the applicant and emailed to the LTFT mailbox at england.ltft.eoe@nhs.net. All submitted forms must be agreed by HEE in order for you to commence your requested reduced working arrangement. 

Please note that LTFT posts cannot always be accommodated straight away.

Please ensure the form is submitted electronically to the LTFT mailbox  with all mandatory fields marked with a red asterisk (*) completed. Hand written, scanned or posted application forms will not be accepted.

Change in plan (Located on the same form)

If you wish to change the percentage of LTFT you are working then you must submit a Change in plan form which is located on the Form 1 document (you will need to use a blank copy).

There is no requirement to renew if you have put your LTFT end date as the same as your CCT date.

Please ensure you save the form locally and open as a PDF with Adobe Acrobat or Adobe Reader before completing. Do not begin completing the form if it has opened as a webpage or in PDF Previewer, as this will disable the electronic signature fields and deem the form invalid. Please see a link to download Adobe Reeder Instructions for setting up a digital ID and additional assistance for Mac users are also available at the bottom of this page. As an organisation we are moving towards going paperless and appreciate your assistance with this. Please ensure you carefully read the instructions on page 1 of the LTFT application form prior to completion.

Important Note: 

You are not eligible to commence LTFT training until you have received written HEE EoE approval for your Form 1: Eligibility Assessment & Training Plan (or the Change in plan form).

Where an application is refused by HEE the applicant has a right of appeal. The training capacity of a training programme and service commitment will have to be taken into consideration. If a doctor in training has had their application to train LTFT declined, they may appeal in writing to the relevant Postgraduate Dean. The Postgraduate Dean’s decision will be final. It is recommended that you first attempt to resolve any issues informally via discussion with your TPD and the AD for LTFT (rowan.burnstein1@nhs.net ).
