The Practice

The practice has been established since the beginning of the century and has always been large, innovative and forward thinking, having 5 partners in 1930.  We now have 9 partners, 4 salaried GPs and a large multidisciplinary team.  We have GPs with interests in clinical areas including orthopaedics, dermatology and ENT.  We have a large nursing team with nurse practitioners, minor illness nurses, chronic disease nurses, nurse prescribers and health care assistants.  We currently have a town centre surgery (South Street) and a surgery on the outskirts of town (Bishops Park) serving our population of 20,000 patients.

The practice has many years experience of training, some current partners have trained at the surgery as FY2 and Registrars.  All partners and staff are passionate about providing high quality teaching and training.  Dr Kwasi Appiah, Dr Sarah Gralton and Dr Sarah Dixon teach 3rd and 5th year students from Barts and the London and final year students from Imperial.  Dr Sarah Dixon is an Honorary Clinical Lecturer for Barts and the London School of Medicine.  The practice also provides training for FY2 doctors attached to the surgery for each 4 Month rotation.

We use EMIS web clinical system and docman for clinical letters.  The practice has an on-line appointment booking system and patients can also request prescriptions on-line.

The Hamilton Practice: A Training, Teaching, and Learning Practice

We are a teaching/training practice for a wide range of health professionals and non-professionals. We strongly believe that by taking on the responsibility for teaching and training others, we further anchor ourselves in the lifelong commitment to provide the very best healthcare, which is evidence based and up to date.

During your relationship as a patient with our practice you are very likely to come across people of a wide range of Health Professions and students whom we are training. We can promise you that your rights as a patient will never be compromised, you will always have the right to refuse to be involved in teaching/training and we absolutely guarantee that nobody being trained will be put in any situation beyond their competence or without supervision.

We are ever grateful to patients for helping us train the next generation of health professionals and other colleagues whom will work alongside us in the National Health Service

We provide learning opportunities for the following:

a) Doctors becoming General Practitioners

We are approved and accredited by Health Education England to be a Training Practice for future General Practitioners. These are qualified doctors with medical degrees but who require training and experience in General Practice to be fully independent and qualified as General Practitioners. They are of varying seniority but are always closely supervised whilst training with us. Typically they stay with us from between 6 and 18 months. They have different titles according to their seniority but are typically known as GP Registrars.

b) Medical Students

We are an accredited teaching practice for undergraduate Medical Students from Barts and the London Medical School. These students are not qualified as doctors and are learning about medical conditions in the community as seen by GP Practices such as ours. They learn primarily by seeing patients, taking histories of patients' symptoms and occasionally conducting examinations.

c) Allied Health Professionals

We also train other health professionals at our practice of all levels of seniority but usually very experienced Nursing Colleagues whom are expanding their skills into areas such as prescribing or some other field of specialisation. Other colleagues may be trainee psychologists, health visitors or student midwives.

d) Work Experience

Every year we are pleased to offer a small number of placements for work experience students to work alongside our administrative staff. They also may sit in some consultations with clinicians as part of their experience, especially if they are hoping to pursue a career in the NHS. They are bound by the same level of confidentiality as any other staff member.

The Limes Medical Centre began GP training in 1955, and since then has amassed considerable experience in the field of medical education. Dr Sidney Scott, a previous senior partner, founded the West Essex VTS Scheme. In the last decade training activities have widened to include both nurse practitioner development and medical student training attachments (linking with Queen Mary's College Westfield and The Royal London and Barts medical colleges). The Limes was a fundholding practice from 1995, and went to PMS in 2001. Following the changes to regional boundaries it is now affiliated to the Eastern Deanery

The team consists of:

Full Time Principals:
Dr Andrew Ashford (lead partner, GP trainer)
Dr Jane Riches (leads medical student teaching)
Dr Tareq Abouharb (Associate Director for Higher Professional Education, London Deanery)
Dr Sanjeev Rana

Part Time Principals:
Dr Susan Hanger (GP trainer)
Dr Katrina Duggins

In addition we have several salaried GPs who contribute to our in-house teaching. We have current capacity for two full time GP registrars. There is a substantial nursing team including two nurse practitioners and one in training, and an attached primary care MacMillan nurse. Our Managing Partner, Debbie Bhodanya, oversees finance and general administration.

Premises

In 1993 we moved from our old premises in Station Road to The Limes Medical Centre which was purpose-built under the cost rent scheme. In 2003 this was substantially extended to provide additional consulting rooms, a dedicated training room and an on-site pharmacy. In addition there are two branch surgeries in Theydon Bois and North Weald, linked to the main surgery by dedicated land lines.

In 2009 we were awarded the contract for administering the Nazeing Valley Health Centre - this is a small friendly nurse-led practice previously run as an APMS provider by West Essex PCT. Nazeing is keen to support training capacity by working with us to provide further variety of experience for our GPStRs.

Clinical Services

The practice provides a full range of primary care services, including antenatal clinics, child development clinics, nurse-run COPD and smoking cessation clinics, minor surgery clinics, and in addition runs secondary care colorectal and diabetes clinics.

Information Technology

The practice is fully computerised with a full local area network linked to both branch surgeries. The clinical system is EMISWEB, running paper-light with hospital letters scanned in. In addition we have Encompass (a computerised full messaging system) running over the network. All clinicians have full NHS Net and Internet access from their desktop, allowing rapid access to core databases (Athens password required). Practice clinical information and protocols are held on a central server allowing easy access from any terminal.

We are currently a five doctor partnership, with four full time partners and one part time partner, in a semi-rural part dispensing, teaching and research practice with an increasing list size, and with an attached salaried GP and two GP Registrars.

Training

The practice acquired training status in August 2006 and within the practice we are lucky to have a GP who is one of the two local GPST Programme Directors and two GPs who have undertaken Associate GP Trainer training.  At present we accommodate one GP Registrar and one Innovative GP Registrar training post.  To support the expansion of our GP training activities our premises have been adapted to enable the practice to take on an additional GP Registrar.

As well as training GPs the practice is involved in the Community Based Medical Education of medical students, working with Queen Mary's College in London to provide training for medical students from Barts and the Royal London Medical School. The medical students join us once a week on a Tuesday afternoon to gain a greater understanding of the role of medicine in Primary Care.

We have been a training GMS practice for nearly 20 years.  We are based in the centre of Buckhurst Hill, London, this is within the M25 motorway.  There are four partners, two who are trainers, we have up to three registrars at any time.  Each registrar has their own room which they keep for the duration of the training.  There are three nurses.  We all work as a team and often go out on social occasions together.  Our practice population is mainly city workers, families and the elderly.  Our half day is on a Wednesday and the surgery is closed in the afternoon.  There are no late evening clinics.  We have a few visits every day and these are equally shared out amongst the GPs.

Initially there is a two week induction period, where the registrars will get to sit in with all the clinical staff and go out with the community nurses.  The registrar will initially start seeing patients at 30 minute intervals and gradually work towards 10 minute appointments closer to their CSA exam.  Tutorials usually take place on a Monday afternoon and last for 3 hours and closer to the exams we run mock CSA cases.  We teach medical students from Barts and the London and Kings and the registrar takes an active role in teaching too.
