What is the Form R?
The Form R is a self-declaration form completed by postgraduate doctors. Until January 2014, trainees were asked to complete an Enhanced Form R for the purposes of registration and revalidation. To avoid duplication and minimise the amount of unnecessary form filling for doctors, this combined form has now been made into two separate parts, Part A and Part B, so that both parts don’t always have to be completed when only one is needed.
The Form R (Part A) asks for personal and contact information that is used to maintain our database. It is requested from you by the HEE EoE Programme and Information Team at registration and then once annually to maintain accuracy of data. A request for this will be sent to you by the member of the Programme and Information Team who deals with your specialty, and any queries about it should be directed to that team member at the email address the request is sent from.
The Form R (Part B) asks for information about your recent scope of practice, time out of training, and declarations relating to revalidation. This information informs the ARCP Panel to comment on your fitness to practice, which in turn informs the Dean’s recommendation to the GMC about your revalidation.
Why do I need to fill in the Form R (Part B)?
The Form R (Part B) is a self-declaration form that is part of the evidence required for your ARCP and revalidation. It is required each time you have an ARCP held for you, even if you are on an Out of Programme (OOP) rotation.
When do I need to fill in the Form R (Part B)?
As part of the revalidation process, your regional deanery or LETB (Local Education & Training Board) will collect a Form R (Part B) from you at certain times during your training programme. For all doctors in the East of England who are in training programmes, HEE EoE will request the Form R (Part B) from you approximately 6-8 weeks prior to your ARCP. This applies for your main annual ARCP and any additional ARCPs, e.g. if you received a 3-month extension in the last ARCP, etc. Times when the Form R (Part B) is collected may vary across regions by local policy.
What happens if I don’t complete my Form R (Part B) in time for my ARCP?
Your completed Form R (Part B) is essential for the ARCP to proceed. In its absence, the panel will issue an outcome 5 and set you a new ARCP date within an appropriate time frame as agreed by the Conference of Postgraduate Medical Deans of the UK (CoPMED). Failure to provide Form R part B may be considered as non-engagement and may lead to NTN removal.
All postgraduate doctors in training are required to submit a Form R. This comes in two parts, which will be requested from you at different times of the year.
Form R (Part A)
This part is the trainee registration form, in which you must confirm your contact details and information about your post. This is to ensure that we have up-to-date information about you. Form R part A is available to complete via TSS. Please see https://heeoe.hee.nhs.uk/faculty-educators/starter-documents for details on how to sign up to TSS. Please ensure that your Form R part A and B are completed on TSS and training agreement signed and returned to Trainee Information System (EOE) <tis.eoe@hee.nhs.uk> prior to starting the training programme.
Form R (Part B)
This part is a self-declaration form, which is used for revalidation purposes for doctors in training. This form will be requested prior to your ARCP. Document is available to download at the bottom of this page.
On your Form R (Part B) you must declare any fitness to practice concerns - that is any significant events, complaints or other investigations you have been named in, and/or any GMC conditions, warnings or undertakings placed on you.
Please note that all new starters from August 2022 must complete the Form R part B for their ARCP via TSS. The previous pdf versions of the Form R part B will no longer be accepted. We are currently working on introduction to TSS for all Postgraduate Doctors in training, starting from every ARCP conducted from January 2023 onwards.
The only exception from TSS introduction will be for Postgraduate Doctors expecting to complete their higher training at their next ARCP. In this case, the pdf Form R part B is acceptable.
We understand that you might have already submitted the pdf version of Form R part B for your recent or upcoming ARCP, nevertheless, please sign up to TSS to submit the online Form R part A and B as a part of sign up process. This will prove to be more efficient way of submitting the Form R part B going forward.

How to use the system
To help get you started we have attached some key information you will need to get you up and running. You will need to set up an account in TSS by following the below sign-up process. You will only need to do this once for the initial set up. Please use the email address you receive the usual ARCP communication from us to sign up. If this is not the email address you want HEE contacting you on, please contact your specialty administrator or tis.eoe@hee.nhs.uk so we can update TIS.
Once you access the system, you will need to submit your ARCP Form R (parts A, B and Covid 19 self-declaration) using the digital Form R, included as a core part of TSS. Please note that you will no longer submit Form Rs outside TSS, if you have any issues, please contact tis.eoe@hee.nhs.uk. Form R part A will be required only once as a part of a first sign up process, irrespective of your current point in training.

- Complete and submit your Form R parts A & B
Please note down the username and password that you created as you will be using this new system to complete all Form Rs going forwards.
